Job Summary

We are seeking a highly skilled Registered Nurse to join our team at IJW Recruitment Solutions Inc. as a Home Care Specialist. This is a permanent position that requires a strong background in nursing and excellent communication skills.

Key Responsibilities
  • Provide high-quality patient care in a home care setting, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop personalized care plans to meet those needs.
  • Collaborate with healthcare teams to ensure seamless care transitions and coordinate with other healthcare professionals as needed.
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and provide emotional support as needed.
  • Maintain accurate and detailed records of patient interactions, including progress notes and medication administration records.
Requirements
  • Bachelor's degree in Nursing or equivalent experience.
  • 1-2 years of experience in a home care or visiting care agency setting.
  • Licensure as a Registered Nurse by provincial or territorial authorities.
  • CPR and First Aid certification.
  • Valid driver's licence and reliable transportation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Preferred Qualifications
  • Experience working with older adults or individuals with complex medical needs.
  • Knowledge of community resources and support services.
  • Fluency in a second language (English is the primary language of communication).
