Job Summary

Serves patients by preparing medications and providing pharmacological information to the multidisciplinary healthcare team. Monitors patient drug therapies and ensures adherence to policies and procedures.

Key Responsibilities
  • Prepare medications for dispensing to patients.
  • Collect and interpret patient information to identify drug-related problems and determine therapeutic outcomes.
  • Oversee drug distribution and ensure compliance with policies and procedures.
  • Apply knowledge when dispensing medication.
  • Review prescriptions for accuracy, appropriateness, and completeness prior to dispensing.
  • Provide patient medication information and education as required.
  • Maintain patient information and ensure removal of outdated, mislabeled, or recalled medications.
  • Credit and return medications to stock.
  • Manage inventory of prescription and over-the-counter medications.
  • Manage third-party payer agreements, reimbursements, and funding sources for medications.
  • Collaborate with Sault Area Hospital staff and programs to support safe transition to community care.
  • Assist with preparation of the drug budget and monitor performance.
  • Document and report any unexpected adverse drug reactions.
  • Function as a professional role model, demonstrating good interpersonal skills and a commitment to self-growth and development.
  • Maintain current knowledge of best practices and benchmarks for service delivery, research, literature, accreditation requirements, policy, and legislative requirements.
  • Ensure security and documentation meet expectations of Ontario College of Pharmacists, NAPRA, and pertinent provincial or federal laws.
Requirements
  • Bachelor's or Doctoral degree in Pharmacy with formal clinical training.
  • Current registration with the Ontario College of Pharmacists in good standing.

  • Computer literacy in a Microsoft environment.
  • Ability to maintain successful working relationships with the interdisciplinary team.

  • Knowledge of medication orders, bio-equivalency, interchangeability of multi-source drugs, formulary, and drug plan management.
  • Knowledge of evidence-based treatment modalities, specifically oncology.
  • Knowledge of relevant legislation.
  • Knowledge of third-party billing requirements and financial management of a retail pharmacy operation.
