We are seeking a skilled Registered Practical Nurse to join our team at ParaMed Home Health Care. As a key member of our community care team, you will play a vital role in delivering high-quality patient care and making a positive impact in the lives of our clients.
Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including assessments, care planning, and implementation of care plans.
- Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
- Conduct regular home visits to assess patient needs and provide ongoing care and support.
- Document patient information and progress in a timely and accurate manner.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a strong commitment to providing excellent patient care.
