Nurse Practitioner

Bayshore HealthCare, a leading provider of home and community health care services, is seeking a dedicated Registered Nurse to join our team. As a Registered Nurse with Bayshore, you will have the opportunity to provide high-quality, patient-centered care in the comfort of clients' homes.
Key Responsibilities:
- Provide skilled clinical care to clients in their homes, including wound care, intravenous therapy, and health teaching.
- Develop and implement individualized care plans to meet clients' unique needs.
- Collaborate with clients, families, and healthcare teams to ensure seamless care coordination.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
Requirements:
- Current registration with the College of Nurses of Ontario.
- Valid driver's license and reliable vehicle.
- Experience in home health care or a related field.
- Strong communication and interpersonal skills.
