The Patient Flow Navigator is dedicated to ensuring that every patient receives the appropriate level of care at the right location and time, delivered by the right healthcare professional. Key responsibilities for this role encompass:

  1. Enhancing patient and clinical outcomes by evaluating and facilitating the efficient management of care, resource utilization, and length of stay.
  2. Identifying and addressing processes or challenges that hinder patient flow from admission through to discharge.
  3. Coordinating the multidisciplinary team's care plan to ensure timely responses to patient needs and consistent communication with patients and their families regarding discharge plans.
  4. Increasing awareness of the patient navigator role and services among the healthcare team to aid in the coordination of patient care and identify patients at risk who may require navigation services.
  5. Guiding patients to become advocates for their own healthcare.
  6. Empowering patients to navigate the healthcare system both internally and externally.

QUALIFICATIONS:

  1. Current Certificate of Registration from the College of Nurses of Ontario is mandatory.
  2. A minimum of five (5) years of recent experience in acute care is required.
  3. Current Non-Violent Crisis Intervention (NVCI) Certificate is necessary (training provided by the Health Centre upon hire).
  4. Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate is required (must be recent, within the last twelve months).
  5. Extensive knowledge acquired through significant involvement in a complex technical or professional field and/or broad experience across multiple fields is essential.
  6. Proven ability to collaborate effectively within a multidisciplinary team environment, including internal and external partners.
  7. Demonstrated strong leadership capabilities are required.
  8. Exemplary facilitation, critical thinking, and problem-solving skills are essential.
  9. Exceptional organizational skills are required.
  10. Experience in data collection and analysis with a focus on accuracy is necessary.
  11. Proven leadership skills and the ability to apply conflict resolution techniques are required.
  12. Capacity to swiftly adjust workload priorities is essential.
  13. Ability to provide leadership during emergency situations is required.
  14. Demonstrated self-direction and the ability to thrive in a dynamic environment are essential.
  15. Commitment to supporting continuous quality improvement initiatives is required.
  16. Comprehensive knowledge of various bed types within the organization and the ability to recommend bed allocation based on unique admission criteria for each unit (e.g., Acute Surgery/Medicine, Complex Continuing Care, Rehabilitation, Assess-Restore Beds, Mental Health, etc.) is necessary.
  17. Good attendance and work history are required.
  18. Commitment to ensuring safety for oneself and others is essential.
  19. Support and contribute to a culture of safety and prevention of adverse health events within the organization.
  20. Bilingualism (English/French) is considered an asset.
