Job Summary

We are seeking a Registered Practical Nurse/Licensed Practical Nurse (RPN/LPN) to join our team of healthcare professionals. As an RPN/LPN, you will play a vital role in delivering high-quality care to our clients, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities

• Provide high-quality nursing care to clients in their homes, as per their individual care plans

• Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies to address them

• Develop and implement individualized care plans, in collaboration with clients, families, and other healthcare professionals

• Administer medications, perform wound care, and provide other nursing interventions as required

• Maintain accurate and up-to-date records of client care, including progress notes, medication administration records, and other relevant documentation

Requirements

• Current registration with the provincial licensing body

• Valid driver's license and reliable vehicle

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Ability to work in a fast-paced environment and prioritize tasks effectively
