Palliative Care Nurse Practitioner

Eden Health is seeking a dedicated Nurse Practitioner to join our team on a Part-Time basis. This role involves working independently while collaborating closely with Physicians to deliver primary medical care to patients facing chronic or life-limiting conditions that necessitate Palliative or Hospice care interventions. The Nurse Practitioner will be tasked with visiting hospitals and skilled nursing facilities, evaluating patients currently under our care, and updating their care plans as needed.

Under the guidance of the Medical Director, the Nurse Practitioner will conduct assessments, review medications, formulate suitable care plans, and provide ongoing evaluation of care. Additionally, this role includes assisting with advanced directives and engaging in discussions regarding goals of care, disease progression, and the grief process with both patients and their families.

Qualifications:
  • Current license from the state Board of Registered Nursing
  • Master's Degree from an accredited Nurse Practitioner's Program
  • National Certification as a Nurse Practitioner or eligibility for certification, which must be obtained within the first six months of employment
  • A minimum of one year of experience as a Nurse Practitioner is preferred
  • Previous experience in Hospice care is advantageous
  • Valid driver's license and reliable transportation are required
  • Ability to pass a criminal background check and national sex offender clearance
  • Capable of performing patient care as necessary
