Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Fraser Health Authority. As a Clinical Pharmacist, you will play a vital role in providing clinical pharmacy services to our patients, including individualized pharmacotherapy, patient medication counselling, and consultation with physicians and other healthcare providers.
Key Responsibilities:- Develop and implement therapeutic plans for patients, taking into account their medical history, current medications, and laboratory results.
- Provide patient education on medication therapy, including benefits, risks, and potential interactions.
- Collaborate with healthcare teams to develop and implement medication therapy guidelines for individual disease conditions.
- Participate in quality improvement initiatives and medication use evaluations to ensure safe and effective medication use.
- Perform drug distribution duties, including processing prescriptions, validating prescriptions, and dispensing medications.
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.
- Strong knowledge of clinical pharmacy practice, medication use evaluation, and research processes and methodology.
- Excellent communication and interpersonal skills, with the ability to work effectively with healthcare teams and patients.
