About the Role

Under the guidance of the Primary Health Care Manager, the Pharmacist will collaborate with an interprofessional team of healthcare providers, including family practice teams and chronic disease management teams, to deliver patient-centered care that ensures safe, effective, and appropriate medication therapy. Responsibilities include:

  • Engaging in selected medical services
  • Identifying and resolving medication-related issues
  • Conducting medication reconciliation
  • Participating in interprofessional medical rounds
  • Providing patient education
  • Making therapeutic recommendations to the healthcare team

The Pharmacist will also oversee drug distribution systems (both inpatient and outpatient) to ensure safe and effective medication therapy, contributing to the goals and objectives of Nova Scotia Health Pharmacy Services.

This position primarily operates from the Yarmouth Regional Hospital but may require coverage at other locations within Yarmouth County as needed.

Qualifications

We invite applications from candidates who possess the following qualifications:

  • Graduation from an accredited Pharmacy program
  • Completion of a Pharmacy Residency Program is preferred
  • Licensed to practice Pharmacy in Nova Scotia
  • A minimum of two years' experience in a clinical hospital pharmacy setting is preferred
  • Strong knowledge of pharmacology and therapeutics
  • Excellent oral and written communication skills
  • Ability to provide accurate and timely recommendations for safe and cost-effective drug use
  • Proficiency in additional languages is an asset, with French preferred

Compensation
