Trillium Health Partners is seeking a skilled Nurse Specialist to join our team at the Queensway Health Centre. As a member of our interprofessional team, you will provide high-quality care to our medically complex continuing care patients.
Key Responsibilities:
- Provide nursing care, teaching, and coaching to patients and their families.
- Assess and classify the physical and emotional condition of patients through reports from staff and interviews with patients and family members.
- Responsible for the provision and management of nursing care for patients in accordance with nursing standards and best practice guidelines.
Requirements:
- Current registration with the College of Nurses of Ontario.
- In good standing with the College of Nurses of Ontario.
- Current BCLS Certificate.
- Ability to perform phlebotomies, tracheostomy care, and IV starts.
- Reliable and flexible individual to work as part of an interprofessional team.
- Experience in Complex Continuing Care, Gerontology, and Long Term Care.
- Ability to think critically and effectively to prioritize needs.
- Highly effective decision-making, problem-solving, and organizational skills combined with excellent interpersonal and communication skills, both oral and written.
- Positive attendance and work record essential.
