We are seeking a highly skilled and compassionate Social Worker to join our team at Dawson Creek and District Hospital. As a Social Worker, you will play a vital role in providing comprehensive assessment, care planning, and treatment coordination for designated clients and their families.

Key Responsibilities:
  • Conduct thorough assessments of clients' needs and develop personalized care plans
  • Collaborate with healthcare providers and the multidisciplinary team to ensure seamless care delivery
  • Provide direct psychosocial care, counseling, and group work to clients and their families
  • Facilitate client self-management and provide education on relevant health and social issues
Requirements:
  • Bachelor's Degree in Social Work from an accredited university
  • Two years of recent experience in a healthcare setting working with medically complex clients with concurrent disorders
  • Current registration with the British Columbia College of Social Workers
  • Valid BC driver's license
