Nurse Practitioner

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at RT Medical. As a key member of our healthcare team, you will be responsible for providing high-quality primary care to our residents in a long-term care setting.
Key Responsibilities- Provide comprehensive primary care to residents, including assessments, diagnosis, counseling, screening, referral, education, treatment, and follow-up.
- Collaborate effectively with physicians and other healthcare professionals to ensure seamless care delivery.
- Develop and implement individualized care plans that address the unique needs of each resident.
- Conduct regular health assessments and screenings to identify potential health issues.
- Provide education and support to residents and their families on health-related topics.
- Participate in quality improvement initiatives to enhance the overall quality of care.
- Develop and maintain strong relationships with residents, families, and healthcare team members.
- Current registration in the Extended Class with the College of Nurses of Ontario.
- Baccalaureate in Nursing (BScN) or Master's Degree in Nursing (MScN or MN).
- Knowledge of Nurse Practitioners' Standards of Practice as set out by the College of Nurses of Ontario.
- Membership with the Nurse Practitioners' Association of Ontario (NPAO).
- Member of the Registered Nurses' Association of Ontario (RNAO) with professional liability insurance.
- National certification in Gerontology an asset.
- Post-graduate certificate and experience in palliative care an asset.
- Experience or a combination of education/training in Geriatrics an asset.
