**Job Summary**
We are seeking a skilled and compassionate Nurse Practitioner, Extended Care to join our team at the Government of Northwest Territories. As a key member of our healthcare team, you will provide high-quality nursing care to residents of extended care and long term care facilities within the Yellowknife Region.
**Key Responsibilities:**
- Provide nursing care to residents in accordance with established standards of nursing practice and the philosophy and objectives of the Northwest Territories Health and Social Services Authority (NTHSSA).
- Work collaboratively with other healthcare professionals to ensure that residents' physical, emotional, psycho-social, spiritual, and daily living needs are met.
- Act as a leader and mentor to other nursing staff, providing guidance and support as needed.
- Triage residents who require medical attention and make decisions regarding the need for physician consultation.
- Develop and implement individualized care plans to meet the unique needs of each resident.
**Requirements:**
- A Bachelor's degree in Nursing or a related field.
- Experience in a long-term care facility, preferably in a leadership or management role.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and other healthcare professionals.
- Ability to work independently and as part of a team, with a high level of autonomy and decision-making authority.
