Job Summary:

The Occupational Therapist is a key member of our multi-disciplinary team at Home and Community Care Support Services North East. We are seeking a highly motivated and caring individual to provide occupational therapy services to our patients in order to maximize their function, independence, and safety in their respective environment.

Key Responsibilities:

  • Provide occupational therapy services to patients in their homes and communities.
  • Assess patients' needs and develop individualized plans to achieve their goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Conduct home visits and assessments to identify patients' needs and develop strategies to support their independence.
  • Develop and implement individualized plans to promote patients' functional abilities and safety.
  • Provide education and support to patients and their families to promote healthy living and independence.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve patient outcomes.

Requirements:

  • Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
  • Experience working in a community/healthcare environment.
  • Comprehensive knowledge of occupational therapy tools, processes, equipment, and assistive technology.
  • Ability to prioritize professional duties, manage multiple patients, and efficiently organize workload.
  • Strong interpersonal and collaboration skills to work with diverse patient groups, healthcare professionals, and community organizations.
  • Effective listening, observation, and facilitation skills.
  • Ability to communicate information effectively through a variety of means.
  • Working knowledge of computer software and Microsoft Office applications.
  • Valid driver's license and access to a vehicle.
  • Advanced oral and written proficiency in English and French.
