Key Responsibilities in Patient-Centered Care:
The Community Pharmacist will ensure the provision of safe, effective, and suitable medication therapy while actively engaging with both employees and customers. This role is vital in achieving excellence in patient care, customer service, and operational efficiency.
- Emphasize clinical services such as medication reviews and disease management
- Develop and implement care plans, ensuring proper documentation
- Assess the appropriateness of prescribed drug therapies
- Monitor patient adherence to medication regimens
- Gather and document pertinent patient, medication, and health information
- Advise on over-the-counter products and nutritional supplements
- Respond to patient inquiries requiring clinical expertise and assessment
- Oversee pharmacy students or provisional pharmacists in restricted activities
- Perform additional duties as necessary
Qualifications & Skills:
- Proficiency in Kroll software
- Experience in a retail pharmacy setting
- Excellent verbal and written communication skills
- Strong organizational abilities with meticulous attention to detail
- Capability to work independently as well as collaboratively within a team
