About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at ComForCare. As a key member of our care team, you will play a vital role in providing high-quality care and support to our clients in their homes.

Key Responsibilities
  • Develop and implement personalized care plans for clients, ensuring their physical, emotional, and social needs are met.
  • Provide skilled nursing visits, including medication management, wound care, and vital sign monitoring.
  • Collaborate with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Conduct thorough assessments of clients' needs and preferences, identifying opportunities for improvement and implementing changes as needed.
  • Maintain accurate and detailed records of client interactions, including progress notes and care plans.
Requirements
  • Graduate of an accredited school of professional nursing.
  • Registered in BC as a Licensed Practical Nurse.
  • Current CPR certification.
  • Access to a reliable vehicle for travel to client homes.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
