Mental Health and Addictions Outreach Nurse Position

Vancouver Coastal Health is seeking a skilled Registered Nurse or Registered Psychiatric Nurse to join our Intensive Care Management team as a Mental Health and Addictions Outreach Nurse in Sechelt, BC.
Key Responsibilities- Conduct comprehensive assessments of clients' mental and physical health needs, identifying priority care requirements and developing effective care plans.
- Provide counseling and support to clients, helping them reduce their reliance on acute and community healthcare services.
- Assess high-risk situations and provide crisis intervention, collaborating with interdisciplinary teams to ensure seamless care.
- Facilitate client movement between acute care, community agencies, and designated health centers, ensuring continuity of care.
- Participate in case conferences and meetings with multidisciplinary teams, promoting effective communication and problem-solving.
- Assist clients with accessing necessary services, such as medical appointments, and provide support with daily living activities.

Qualifications- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Diploma in Nursing or Psychiatric Nursing.
- Two years' recent experience working with individuals with mental health and addictions issues, or an equivalent combination of education and experience.
- Valid BC Driver's License, with local area travel requiring the use of a personal vehicle.
