- Conduct home visits to assess client needs and provide education on self-care and disease management.
- Document client information and progress in a timely and accurate manner.
- Participate in ongoing education and professional development to stay current with best practices and industry developments.
- Current registration with the provincial or territorial nursing regulatory body.
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a team.
