Job Summary

The Diabetes Health Educator supports the health and wellness of Chippewas of the Thames First Nation community members through the delivery of culturally relevant diabetes education and prevention programming. The role equips individuals with the knowledge, tools, and confidence to prevent or manage diabetes through holistic, evidence-based education and healthy lifestyle support. The Diabetes Health Educator ensures program objectives are met while promoting community-focused care and positive health practices.

Responsibilities

Diabetes Education

- Conduct holistic health assessments, and support and monitor clients living with diabetes.

- Provide one-on-one and group education on nutrition, physical activity, and blood sugar monitoring.

- Screen, educate, and refer school-aged children and community members for early detection and diabetes management.

- Coordinate monthly Foot Care Clinics and ensure client follow-up.

- Develop and distribute educational materials (newsletters, social media, displays).

- Conduct home visits (as approved) to assess client needs.

- Research diabetes trends and identify service gaps.

Programming

- Plan, deliver, and evaluate diabetes education programs, support groups, and community initiatives.

- Provide presentations on healthy eating, hydration, and traditional wellness teachings.

- Facilitate healthy food and cooking classes with the Dietitian.

- Host monthly diabetes education sessions and integrate cultural teachings into all programming.

- Monitor program effectiveness and implement improvements as needed.

Administration & Collaboration

- Maintain accurate records and submit required reports and financial documentation.

- Work closely with Community Health Nurses, the Health Promotions Worker, and other health professionals.

- Network with schools, local health units, and service providers to enhance prevention strategies.

- Participate in professional development and maintain current certifications.

Qualifications

1. Certified Diabetes Educator (CDE) designation with the Canadian Diabetes Educator Certification Board (CDECB) required.

2. College Diploma or higher in Nursing, Nutrition, Health Promotion, Community Health, Indigenous Community Care, or a related field.

3. Minimum two (2) years of experience working with Indigenous communities in health, wellness, or education settings.

4. Foot Care and Indigenous Diabetes Support Program certifications are strong assets.

5. Knowledge of Chippewas of the Thames First Nation culture and traditions.

6. Proficiency in Microsoft Office, EMRs, and social media for health promotion.

7. Current certifications (or willingness to obtain):

- First Aid/CPR

- WHMIS

- Mental Health First Aid

- SafeTALK

- ASIST

- Safe Food Handling

- Privacy and Cultural Safety Training

8. Valid driver’s license, insurance, reliable vehicle, and current Vulnerable Sector Police Check.

9. Flexibility to work evenings and weekends when required.

10. Preference will be given to First Nation applicants as per Section 16(1) of the Human Rights Act.
