Joining SE Health was the best career decision I’ve ever made. The people are really nice, and I was free to pick up my children after school every day. I’ve also had many opportunities to grow and practice to my full scope. – Mary, 20 years at SE Health.
We are seeking dynamic, passionate Nurses, who want to be part of the transformational change in home and community care by providing care in a truly client-centered approach.
SE Health provides palliative care services that support both the individual facing a life-threatening illness and his/her family. Improving the quality of life is a key focus of the care that is delivered by our team of nurses working with other professionals within the health care system (personal support workers, physicians, chaplains, therapists, hospice volunteers, etc). Recognizing that individuals living with illness are also faced with many needs, our staff is able to integrate the physiological, psychological, socio-cultural, and spiritual aspects of care to assist those coping with their situation. Our community-based palliative care team offers a support system to help people live as actively as possible until death and to assist families during the person’s illness and during bereavement.
Responsibilities:
- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
- When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Why join our team?
- Competitive compensation. Our Total Rewards package includes a competitive salary, group benefits, RRSP pension, on demand pay, car loan support, supportive housing and exclusive perks/discounts available only to SE Health staff
- Develop yourself with SE. We have education bursaries, Tuition Assistance Programs, continuing education, training, and professional development to support your lifelong learning
- Grow your career. SE Health is a large national inclusive organization with diverse healthcare business lines. You’ll have an opportunity – and will be supported – to do different types of patient-facing nursing roles in different locations if that is what you’re interested in. Or you can pivot to supporting or leadership positions where you use your nursing skills as a foundation to contribute and make an impact in different ways.
- Manage your life. At SE, you’ll have the flexibility to manage your schedule to balance your life. You’ll be supported with the time you need to meet the needs of your clients, meet your own needs, develop yourself and your career, and be part of a team
- Paid mileage between clients.
- Mobile device and 3 uniforms provided
