As a key member of the interdisciplinary team at SOAHAC, the Harm Reduction Care Coordinator will offer client-centered services. This role involves care coordination, advocacy, and system navigation while promoting harm reduction practices. Ideal candidates are passionate about Indigenous wellness and have a strong desire to assist clients facing complex health issues.
Key Responsibilities
• Provide harm reduction support and care coordination
• Conduct intake assessments and wellness planning
• Promote health education and overdose prevention
• Collaborate with health service partners
• Build relationships with Indigenous communities and organizations
Requirements:
• Post-secondary degree in health or related field
• Minimum one year of relevant experience
• Knowledge of Indigenous harm reduction philosophies
• Strong advocacy and relationship-building skills
• Ability to work independently and in a team Contribute to the well-being of Indigenous communities with your expertise in harm reduction and advocacy at SOAHAC.
