Key Responsibilities
1. Deliver high‑quality, patient‑centered care in collaboration with our interdisciplinary team.
2. Conduct comprehensive assessments, diagnoses, and treatments for a diverse patient population.
3. Develop and implement personalized care plans, ensuring continuity of care.
4. Engage in health promotion and disease prevention strategies to improve patient outcomes.
5. Maintain accurate and timely documentation of patient interactions and care provided.
6. Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.
7. Provide mentorship and guidance to nursing staff and students.
Requirements/Qualifications
1. Current registration as a Nurse Practitioner with the College of Nurses of Ontario.
2. Master’s degree in Nursing or equivalent education and experience.
3. Proven clinical experience in a healthcare setting
4. Excellent communication and interpersonal skills, with a compassionate approach to patient care.
5. Strong organizational skills and the ability to work independently and collaboratively.
6. Commitment to ongoing professional development and continuous learning.
