As the Homecare Nurse (HCN), you'll join an interprofessional team dedicated to comprehensive community health. Key Responsibilities
• Assess and treat clients in their home environments
• Collaborate with health professionals for comprehensive care
• Develop and track personalized care plans for clients
• Support intake and maintain accurate medical records
• Actively participate in organizational meetings and committees
Requirements:
• Current Registered Nurse registration in Ontario
• Experience in home care preferred
• Knowledge of Indigenous health and cultural competency
• Willingness to work collaboratively and learn
