What You'll Do:
- Provide ongoing support to individuals in their community to help them function independently and access mental health services.
- Engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
- Assist consumers in accessing needed services and ensure transportation to appointments.
- Develop individualized service plans and ensure the consumer's engagement with community mental health and non-mental health services.
- Assess the need for crisis intervention and assist in psychiatric emergency situations.
- Encourage wellness and recovery while maintaining individual dignity and respect.
- Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.
Job Requirements:
- Bachelor's Degree in Social Work, Psychology, or a related field from an accredited college or university.
- Experience with community-based mental health services is preferred.
- Working knowledge of computer software, including word processing and Excel.
- Valid Driver's License and an acceptable driving record.
- Strong verbal and written communication skills.
- Ability to work effectively with individuals and teams.
- Compassionate, client-centered approach to care.
