Regional Nurse - Palliative & End of Life Care page is loaded## Regional Nurse - Palliative & End of Life Carelocations: IUIH Head Office Main 1time type: Part timeposted on: Posted 4 Days Agojob requisition id: JR100011**Join an Organisation dedicated to Closing the Gap in Indigenous Health**The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.The Primary Health Care Unit has responsibility for direct delivery of primary health care services for Aboriginal and Torres Strait Islander people in specific settings, on behalf of the IUIH Network.The position will work within the Palliative Care Services Team and assist in the implementation of a new service model designed to meet the unique needs of Aboriginal and Torres Strait Islander individuals and their families. As part of multi-disciplinary team, the position ensures Palliative Care and End of Life care, services, training, and information are delivered in accordance with the IUIH Model of Care. We are transitioning to a 24/7 support service, and this role will be expected to support in this capacity.**Your day-to-day duties may include:*** Provide testing, treatment, and care under the direction of the GP including routine wound care, administration of prescribed medications, testing and monitoring procedures such as ECG, spirometry, BSL measurement, urinalysis, and other activities within recognized scope of practice.* Attend home visits where required and under the direction of the Palliative Care and End of Life Care Team, Clinic Practice Manager and / or Senior GP.* Work closely with the Palliative Care and End of Life Care Team and IUIH Network providers to support regular tracking of progress for disease monitoring, care and supporting the implementation of changes in response to identified gaps.* Developing palliative care, end-of-life care and voluntary assisted dying tools, training, resources, and information to support one-to-one discussions with clients, their families and with IUIH and Member Services.* Enter any recalls and reminders issuing from the consultation into the electronic health record system.* AHPRA Registration* Current CPR Certificate* Minimum Diploma of Nursing* Ability to support the team in this 24/7 support service**Desirable, but not mandatory:*** Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities*At IUIH we recognise that we operate in a diverse community and welcome and encourage applications from all ages and genders, Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, the LGBTIQ+ community, veterans, refugees, and people with disability.** At IUIH ‘Our Collectivity is Our Strength’.* We are a diverse and inclusive workplace that invites applications from all people.* Be part of Australia’s largest community controlled Aboriginal Health Service.* Access to “Growing Our Own Opportunities” through in-house training and programs.* Weekly Staff Wellness activities and all staff ‘Gather and Grow’ yarns and award ceremonies.* Opportunities to attend community events such as NAIDOC, Rugby League Carnivals and more.* Onsite Presto’s Café at our Windsor Office providing a range of hot and cold beverages and healthy breakfast and lunch options.
