Vancouver Coastal Health seeks an experienced Social Worker for the Southeast Urgent and Primary Care Centre. You will play a critical role in assessing, planning, and executing social work care for patients facing urgent health issues. This team-based approach ensures comprehensive care for patients and families in need.
Key Responsibilities
• Conduct psycho-social assessments and develop care plans
• Foster partnerships with local primary care providers
• Support families dealing with chronic illness and mental health
• Implement harm reduction and strengths-based care techniques
• Collaborate with an interdisciplinary team for patient success
Requirements:
• Accredited Master's Degree in Social Work
• Two years clinical experience in a relevant setting
• Current registration with the British Columbia College of Social Workers
• Strong ability to conduct mental status exams
• License for local travel between care sites Utilize your skills in this impactful social work role to provide urgent support and guidance at Vancouver Coastal Health.
