The nurse practitioner provides primary health care to clients at ConnectWell Community Health. This ongoing position is located primarily at the Lanark site and Carleton Place satellite site locations. ConnectWell Community Health provides a comprehensive benefit package and the option to join HOOPP (Healthcare of Ontario Pension Plan). Apply todayPosition Summary:The Primary Care Nurse Practitioner (NP) functions as a member of the Primary Care Team in the provision of primary health care services offered at ConnectWell Community Health. Primary health care services are provided at ConnectWell and in the community, with an emphasis on health promotion, client education and self-management. The RN (EC) must be aware of, understand, and adhere to the Standards of Practice set out by the College of Nurses of Ontario.

- Completion of recognized Primary Care Nurse Practitioner program
- Masters degree in Nursing, or equivalent recognized education and experience
- Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario
- Two years experience in the nurse practitioner role in a primary care setting or community preferred
- Demonstrated ability to work effectively in an inter-disciplinary team
- Demonstrated leadership in promoting a shared responsibility for professional practice and development
- Ability to work evening hours, as required
- Ability to provide on-call telephone services on a rotation basis
- Valid driver’s license and use of a vehicle

Reporting Relationship: The successful candidate will report to our Senior Manager, Primary Care
