Compensation: Starting from $141,494 – $160,312 plus comprehensive health benefits. Dedicated time for non-clinical work. Relocation benefits available.
Location: This position is required to work onsite at 625 Powell Street, Vancouver,
BC Centre for Excellence in HIV/AIDS.

Organization
The BC Centre for Excellence in HIV/AIDS (BC-CfE) is Canada’s largest HIV/AIDS research, treatment and education facility nationally and internationally recognized as an innovative world leader in combating HIV/AIDS and related diseases. A multidisciplinary team of Clinicians, Laboratory Scientist , Health Economists, Epidemiologists, Clinical Researchers, Statisticians, Programmers, and Data Analysts work collaboratively to improve the health of British Columbians living with HIV and other chronic medical conditions.
The BC-CfE opened the Hope to Health Research and Innovation Centre (H2H) in late 2019 and provides interdisciplinary primary care services in Vancouver’s Downtown Eastside. H2H’s team-based approach to primary health care is based on the best evidence for service design in engaging marginalized populations.
Program
The Downtown East Side Hope to Health IntegratedComplex Primary Care(H2H) clinic provides an innovative care model with low threshold, client centered, proactive, highly coordinated, comprehensive complex primary care.
The clinic aims to work with clients who have complex medical or social needs in the Downtown East side community, to prevent and/or improve outcomes related to HIV, Hep C, mental illness and substance use and other chronic illnesses.
The H2H Centre includes harm reduction programs and selected specialty services, as dictated by the specific medical profile of the clients enrolled. Medical and psychosocial/behavioural interventions and services are coordinated and provided by a functionally integrated interdisciplinary team. The interdisciplinary clinical team work at the Hope to Health Clinic, is enhanced by research through ongoing monitoring, evaluation, and quality improvement, as well as promoting knowledge translation.
Role
Hope to Health primary care clinic is recruiting for Nurse Practitioners who will be part of a supportive multi-disciplinary care team that includes an RN, LPN , social worker, peer support worker, and MOAs, as well as in house access to other health providers: Family physicians, PT, OT, dietitian, psychologist, clinical pharmacist, phlebotomist and psychiatry consulting services. This opportunity is a 0.3 to 0.5 to full time equivalent, although full time position is preferred. The patient population is mainly complex clients living in the DTES with many co-morbidities, including mental health, substance use disorders addiction, and psychosocial issues. The clinic supports learning, skill development and continuous quality improvement through clinical simulation exercises and a range of quality improvement initiatives.
Responsibilities
- Diagnoses and treats previously undiagnosed clients for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner’s scope of practice; writes orders for treatment and medications; provides first line care in emergencies.
- Monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment.
- Establishes priorities for management of health, diseases, disorders, and conditions; provides follow-up treatment; communicates with clients and families about health findings, diagnoses and priorities, outcomes and prognoses; supports and counsels clients in their responses to diseases, disorders and conditions.
- Collaborates and consults with physicians or other health care and social service providers as appropriate to assess and diagnose client status. Develops and implements treatment plans.
- Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
- Participates in research contributing to improved client care and advances in nursing, health policy development and population health.
- Maintains population health focus by implementing screening and health promotion activities for populations at risk.
- Other duties as required.
Please note, H2H clinic does not provide the following services, as they require a different set and level of competencies in order to provide safe care:
- Prenatal
- Postnatal
- Pediatrics (13 and under, or in exceptional circumstances)
Schedule
- 8 hour shift
- Monday to Friday
Qualifications, Education, Training and Experience
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM) plus 4 years of recent, and relevant clinical nursing experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE) required.
- Completion of the Basic Cardiac Life Support Course, Level C.
- Ideal candidate will have 2 years of work experience as an NP.
Skills & Abilities
- Recent relevant clinical nursing experience or interest in substance use care. Comfort and ability to support clients with addiction medicine care needs.
- Demonstrated knowledge and commitment to trauma informed care and culturally safe care.
- A strong preference will be given to those with expert knowledge of inner-city primary care and substance use disorders.
- Ability to work alongside and in partnership with colleagues to help the Primary Care team achieve its aims and objectives.
- In good standing with the BC College of Nurses and Midwives(BCCNM).
