JOB SUMMARY

Position duties include comprehensive assessment and developing individual recovery plans, providing counselling, crisis intervention, harm reduction education, provision of harm reduction supplies, and case management for people living with mental health and addictions as a member of the Mental Health & Addiction Services multidisciplinary team. The position involves moderate physical activity, including the provision of practical assistance for clients in relation to independent activities of daily living. This position provides intensive services to individuals according to a housing first approach through assertive, housing-focused case management in various rural communities throughout Grey Bruce.

KEY DUTIES – RESPONSIBILITIES

- Provide episodic and ongoing mental health and addiction care
- Conduct comprehensive psychosocial assessments using required assessment procedures and instruments following recovery values and principles
- Formulate treatment plans using internal and external consultation as appropriate
- Provide treatment services using various modalities including motivational interviewing, solution-focused therapy, CBT and DBT
- Deliver harm reduction strategies based on best practices and guiding principles
- Provide case management support, utilizing knowledge of community resources, local landscape pertaining to social issues, homelessness, mental illness and addiction to deliver social supports based on client-directed goals and needs
- Advocate on behalf of client for access to resources related to social determinants of health, such as income and social protection, education, food security, housing and basic amenities, social inclusion and quality health services
- Establish effective therapeutic relationships following harm reduction and recovery values and principles
- Responding and offering support during opioid-related overdoses
- Engage positively with people who use illicit substances and other stigmatized populations using a harm reduction approach
- Support clients to safely and successfully live independently, including providing support with Instrumental Activities of Daily Living

Hours of work are 37.5 hours per week. Current shifts are 10 hours in length (subject to change), including a 30-minute unpaid lunch break. Weekend shifts will be required. Some evening work may be required.

- A competitive hourly wage ranging between $37.86 and $41.80 (5 step grid)

What you will bring

- A degree in the healthcare field with additional training in mental health, addictions, and harm reduction
- A minimum of three years recent experience in mental and addiction services
- Thorough understanding of individual recovery and client-centered service delivery
- Experience in providing harm reduction education and counselling and distributing harm reduction supplies
- Thorough understanding of harm-reduction, trauma informed practice and Housing First philosophy
- Experience in dealing with persons victimized by crime such as domestic violence, sexual assault, human trafficking, suicide, homicide, and sudden deaths
- Familiarity with psychotropic medications and experience in assisting clients to manage their medications and their primary health-care needs is required
- Demonstrated skill in assessment, crisis management and counselling
- Strong team skills and ability to work well with community and professional groups is essential
- Ability to work well under pressure
- A valid Canadian driver’s license and the use of a reliable personal vehicle
- Minimum $2,000,000 third party liability vehicle insurance
- A satisfactory police record check including vulnerable sector screening
