Summary
The Housing Based Case Manager (HBCM) is responsible for providing support and system navigation with the purpose of developing life skills and personal well-being for individuals & families residing within the YWCA Supportive Housing programs, and across Elgin County to improve housing stability. The HBCM will support individuals & families by providing system navigation, referral, goal setting meetings and orientation. Duties will include, but are not limited to completing intakes, SPDAT's, tours and move-ins. They will also meet one-on-one with individuals using motivational interviewing, attending case conferences, providing life skills workshops, conducting housing program reviews, completing monthly monitoring checklists, and chairing monthly house meetings.
Duties will include, but are not limited to, attending regular meetings, attending professional training workshops, and continuing education to remain current with trends and best practices in the industry. The HBCM will work in the central office and across all areas of Elgin County.

Job Duties
- Communicate with individuals in a manner that shows respect for their life experiences and goals.
- Make comprehensive referrals and "warm" transfers as appropriate for the individuals.
- Coordinate and facilitate meetings with community resources to support the unique needs of individuals/families.
- Conduct program intakes and orientation to individuals moving into YWCA housing.
- Complete SPDAT upon move in and regularly thereafter.
- Support individuals with goal setting based on their strengths.
- Support individuals in establishing long-term housing goals.
- Liaise with landlords and build relationships to further housing stability.
- Provide housing-based case management to individuals/families moving from YWCA housing into community.
- Promote and encourage everyone to maintain an optimum level of health, well-being and self-esteem.
- Identify barriers individuals are facing, and secure resources to achieve identified goals.
- Support individuals with problem solving compatibility, resiliency and conflict resolution.
- Ensure timely completion of case management notes and provide data to Homelessness System Developer as needed.
- Coordinate events and conduct life skills activities.
- Attend bi-weekly case management meetings.
- Attend regular staff meetings and team meetings.
- Work in tandem with Housing Team to deliver comprehensive, quality programming.
- Attend bi-weekly meetings with Manager of Housing to communicate program non-compliances, facility upkeep, and maintenance.
- Assist the Manager of Housing with rent arrears and program non-compliance notifications.
- Respect and protect the rights of individuals, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity; and their right to live free from retaliation, discrimination, and abuse.
- Respect and protect the privacy of information relating to all company clients, employees, and business affairs.
- Maintain and model appropriate personal boundaries with individuals/families.
- Understand and uphold the philosophy of the YWCA St. Thomas-Elgin.
- Maintain current knowledge and practice of all relevant plans, policies and procedures as established by the company.
- Other duties shall be assigned as required.
Requirements
- Post Secondary degree or diploma in Child and Youth Work, Social Work, Addictions Counselling, or relevant field required.
- More than 4 years' experience working with vulnerable populations preferred.
- Previous community related experience a definite asset.
- Working knowledge of housing supports and applications.
- Demonstrated knowledge of harm reduction, diversion, and theories of change.
- Able to effectively communicate both verbally and in writing.
- High level of critical and logical thinking, analysis, and reasoning to identify underlying principles, reasons, or solutions.
- Proven organizational, time management and prioritizing skills.
Work Conditions
- May be exposed to infectious waste, disease, and conditions.
- Interacts with adult, youth, families, employees, management, and the public at large.
- Intermittent physical activity including walking, standing, sitting, and lifting.
- Manual dexterity required to operate computers and peripherals.
- Overtime may be required.
- Must have own transportation to work in a variety of locations across Elgin County.
