These roles support patients admitted into acute care at St. Paul's Hospital who are followed by the Addiction Medicine Consult Team (AMCT), providing point-of-care nursing education, substance use and overdose education, and care planning, working closely with physicians, other nurses, pharmacists, social workers, peers, and other partners to strengthen harm reduction-focused care across the hospital, as well as clinical support for the Overdose Prevention Site. - Support complex care planning and coordination: Partner with teams to develop and implement care plans and behavioural support plans, identify strategies for patients with complex needs, and help translate acute-specific policies and practices into day-to-day care.
- Connect the hospital, AMCT and harm reduction services: Triage and support AMCT referrals, collaborate closely with the interdisciplinary peer team, provide clinical support to the Overdose Prevention Site, and strengthen communication and continuity across services.
