Reporting to the Manager of Specialized Programs, the Nurse Practitioner (NP) provides primary health care with a strong emphasis on health promotion and disease prevention within the scope of practice and standards of care outlined in the Expanded Nursing Services for Patients Act and Regulations and Practice Expectations Specific to Registered Nurses in the Extended Class (NPs). The NP diagnoses and manages common health problems and diseases both independently and in collaboration with the client and other providers within the circle of care. The NP works as part of an interdisciplinary team to set priorities, develop, implement, monitor, evaluate programs and address issues in the community that affect health. The NP is responsible and accountable for their practice.
Service Responsibilities:
Health Assessment and Diagnosis:
- Provides on-going, comprehensive primary health care to clients of all ages within the scope of an NP, including assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up.
- Assesses each client to determine the presence of existing and potential health problems, with a significant focus on physiological lifestyle, psychological, social-cultural, spiritual and environmental risk factors that may influence the client’s health status. The NP deals with these personally or by referral to or in consultation with other service providers and programs in the community.
- Determines the need for, orders and interprets screening, and diagnostic tests.
- Keeps complete, accurate and timely records of client visits within the electronic medical record (EMR), which will provide information to assist other practitioners in continuing client care.
- Collaboratively with the client, establishes a plan of care and as appropriate with the clients and their families, encourages increased responsibility for their health by involving them in risk factor and health problem identification, goal setting and the choice of interventions for health problem treatment, prevention and health promotion.
- Sees clients at the Centre with scheduled and urgent appointments, in the community through outreach and home visits, during regular and extended hours.
Forms Completion:
- Discusses need for completion of forms either brought by the client or initiated by the Nurse Practitioner to further support the client in reaching their goals towards health and social determinants of health.
- Initiates and manages the care of clients and/or monitors the on-going therapy of clients with chronic stable illness by providing currently sanctioned pharmacological interventions, appropriate complementary and counselling interventions.
- Prescribes drugs as currently designated.
Community Health:
- Synthesizes information from individual clients to identify broader implications for health within the family and the community.
- Counsels individuals and families on health-related and lifestyle issues.
- Promotes awareness of the Centre’s programs and participation by the community.
- Initiates and maintains partnerships with relevant community agencies and professionals.
- Develops and maintains a client reference library and actively participates in the preparation and distribution of materials for client education.
- Participates in the development, implementation, monitoring and evaluation of education, counselling and health promotion programs and services including community initiatives.
- Uses group facilitation skills to lead community programs and/or focus groups.
- Provides support and supervision to students who have placements at CHCN.
Organizational Responsibilities:
- Collects statistical information as required by the Centre.
- Incorporates critical thinking practice and a collaborative approach to problem solving and decision making.
- Integrates into work functions the principles of respect and value for staff, clients, and the community.
- Works in a manner that preserves confidentiality. Adheres to the Personal Health Protection Policy and the Confidentiality Policy for the Centre.
- Works in a manner that seeks to minimize risk to clients, self, the Centre and to staff. Committed to safety, works in a manner that demonstrates responsibility for following the Centre’s safe work practices and works in compliance with the Occupational Health and Safety Act. Takes responsibility for and is expected to report and unsafe/at risk conditions as well as be accountable for their correction.
- Communicates in a timely and positive manner with the Clinical Director regarding job content, specific objectives and personal performance. Identifies performance goals, objectives and learning needs. Identifies concerns in a timely manner and, in collaboration with the Clinical Director, seeks resolution.
- Contributes to the overall mission of the Centre by participating in meeting and on committees to identify, respond, and/or resolve issues or opportunities for improvement.
- Incorporates and contributes to the Centre’s efforts for continuous quality improvement by identifying, implementing and evaluating policies and practices to support best practice.
- Incorporates and strengthens collaborative and interdisciplinary teamwork.
- Participates as a team member in all staff functions: program planning, team and inter-team meetings, all staff meetings and team building.
- Performs other duties as assigned.
Professional Development:
- Maintains and develops professional competence through on-going professional development. Fully participates in the Quality Assurance “Reflective Practice” of the College of Nurses of Ontario.
- Stays current and aware of opportunities to implement new, evidence-based methods of client assessment and treatment.
- Participates in self-directed learning to ensure that practice remains relevant by attending professional conferences, e-learning and journal reviews.
- Successful completion of an educational program for Nurse Practitioners – Primary Health Care stream preferred.
- Certificate of Registration from the College of Nurses of Ontario with Registration in the Extended Class with annual renewal.
- Professional liability coverage.
- Knowledge and understanding of primary prevention, health promotion theory and practice.
- Knowledge and experience in working with a diverse population who could be marginalized and vulnerable.
- Proficiency in the use of computers and various software applications.
- Holds current Basic Life Support for Healthcare Providers certificate.
- Demonstrated ability to work effectively within a multi-disciplinary team.
- Valid Ontario drivers’ license and use of vehicle required
- Ability to work flexible hours, if required, including on‑call services.
Salary: $124,621.90 per annum
Hours: 35 hours per week
Hours of Work: Days, rotating on‑call schedule
