ABOUT the HART HUB
The HART Hub Sault Ste. Marie is a community driven response to the escalating challenges of homelessness, addiction, and mental health in our region. Rooted in compassion and collaboration, the Hub will serve as a centralized, accessible, and inclusive space that supports the entire community from the most marginalized individuals to frontline service providers, community organizations and residents impacted by this crisis.
This initiative is grounded in the understanding that these complex social issues affect everyone, directly or indirectly. By creating a hub that integrates health, housing, and social services, we aim to reduce barriers to care, improve outcomes and foster a more resilient and connected community to support those in need.
The goal is for HART Hub service hours to evolve to a 24/7 operation. However, initially service hours at the main hub location will be 7:30am – 9:00pm ensuring the hub remains central and available to all mobile, satellite, and outreach staff, and community members requiring supports. Outreach and mobile services will end at 8:00pm to allow time to return to the main hub location, complete administrative tasks, and debrief. Services at the main hub location will stop at 8:30pm to allow for administrative tasks and end of day debrief.
The overall goal of the HART Hub is to establish a community wide interdisciplinary team that can respond and support the complex needs of individuals and the community, helping address the housing, mental health and addiction crises.

POSITION SUMMARY
The Case Manager is responsible for the delivery of case management services to individuals experiencing mental illness disorders, intellectual disabilities and/or substance use disorders by assisting clients in connecting them to resources that can considerably improve the quality of their lives and increase their overall wellbeing. The position is community-based, requiring Case Managers to meet clients within their environments. The Case Manager is responsible for assessing clients’ needs and support systems; planning for crises and helping clients develop coping mechanisms; explaining these scenarios and their attendant concerns with compassion; and coordinating and monitoring of the client’s use of these services. The Case Manager carries out these responsibilities in accordance with the mission, vision and values of CMHA Algoma, relevant legislation and regulations and organizational policies, to promote the delivery of efficient and high-quality services.
KEY RESPONSIBILITIES
- Delivers case management services for individuals with serious mental health conditions and/or substance use disorders as well as their significant others.
- Specific areas of support may include the following based on individual needs: housing/home maintenance, food security, self-care, physical health, harm reduction, diagnosis and treatment, financial, transportation, legal, and varying daytime activities.
QUALIFICATIONS
Education, Certification & Experience
- A university degree in a Human Services related field
- Current registration with OCSWSSW or a regulated body under the Regulated Health Professions Act, preferred
- Demonstrated knowledge of community resources and organizations
- Certification in First Aid/CPR
- Certification in ASIST (or willing to obtain on own within 3 months)
- Minimum three years experience in the fields of mental health and addictions with an emphasis in community-based services
- Knowledge and experience in conducting bio-psychosocial assessments
- Experience in the criminal justice system and knowledge of community resources
Knowledge, Skills and Abilities
- Ability to work in partnership with clients
- Extensive knowledge and understanding of the mental health and addictions services
- Extensive knowledge of standardized screening and assessment tools
- Extensive knowledge of community services, supports and resources
- Ability to apply Recovery principles and empowerment-orientated philosophies and practices in work with clients
- Demonstrated initiative, dependability, creativity, and attention to quality of work skills and abilities
- Ability to demonstrate diplomacy and negotiation skills in client and system advocacy
- Ability to make sound clinical judgments
- Excellent organizational and time management skills
- Excellent written and verbal communication skills
- Ability and flexibility to work independently and as part of a team
- Excellent computer skills and competency in software applications used by the agency
- Demonstrated ability to engage in reasonable self-care strategies that incorporate balancing work/life responsibilities
- Excellent problem-solving and decision-making skills
Other
- Acceptable Criminal Record Check and Vulnerable Sector Check upon hire and to be maintained throughout employment
- Ability to provide effective client services in English and French, is preferred
- Able to meet the essential physical requirements of the position including ability to sit for prolonged periods
- Valid driver’s license, proof of insurance and access to a reliable vehicle
