The Clinical Care Partner role will play a pivotal part in our Alberta growth strategy, helping to shape and enhance the care experience for our residents and supporting our dedicated teams on the ground. As a trusted advisor and key member of our team, the Clinical Care Partner will provide expert guidance and support to enhance the quality of care across our Alberta homes, ensuring our commitment to resident-centered care and clinical excellence.
**Please Note**: This role is required to have on-site presence at our four sites across Alberta - Edmonton, FT Saskatchewan, Medicine Hat, and Airdrie. Travel to our Long Term Care sites is required. All Travel will be reimbursed.
As a Clinical Care Partner, your typical day will impact the following ways:
- In collaboration with the Vice President of Operations (VPRO), General Manager, and Director of Care, provides follow-up and support to community team members in order to ensure that care meets and/or exceeds the organization's and provincial health authority requirements.
- Influences culture and change management through collaboration, building positive relationships with internal and external stakeholders to support joint problem-solving and align with strategic directions.
- Works with the Director of Care to implement staffing models that reflect evidence-based practices and enhance the quality of care and services for residents and team members.
- Leads community and organization-wide improvement processes/projects as directed and in consultation with the VPRO and community team members.
- Conducts quality assurance activities to determine effectiveness of services provided and assists in setting directions for change.
- Collaborates with community leaders to address specific clinical areas (i.e. falls, skin & wound, pain management, etc.) and facilitates improvement through a train-the-trainer model.
- Conducts scheduled onsite visits to the communities to support leaders with day-to-day clinical operations. Provides subject-matter expertise for the development of clinical policies and procedures based on current evidence.
- Participates in the orientation of nursing leaders.
- Works with the community leaders to navigate clinically complex situations to a desirable outcome.
- Works in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
- This role requires the successful candidates to travel to our long-term care sites to support operations.
Education and Experience Required:
- Must be registered as a Registered Nurse (RN) in good standing with the applicable provincial professional college.
- Post Baccalaureate education in Nursing, Health Sciences, or related field is preferred.
- Preference will be given to those candidates with Director of Care experience.
- Certificate of Gerontological Nursing (Canadian Nurses Assoc.) preferred.
- Knowledge and understanding of provincial regulatory requirements and inspection process.
- 3 years related management and relevant experience in long-term care.
- Demonstrated leadership, organizational, and team building skills.
- Evidence of ongoing professional development and a demonstrated commitment to continuous learning.
- Highly motivated; demonstrates an ability to be self-directed, working independently and as a member of a collaborative team.
- Able to drive and/or travel to care communities - access to a reliable vehicle is required. ** Travel Reimbursement Applies**
