Seaway Valley Community Health Centre is a charitable, not-for-profit community-led organization. SVCHC targets issues impacting the local community’s health through health promotion and community development programs.
We provide a wide range of health and wellness services and programs to families and individuals who live in the communities we serve. We deliver care in many ways: one-on-one, personal development groups and community initiatives. We believe that our collective efforts and partnerships with key agencies contribute to improving both individual and community health by identifying and addressing underlying conditions. We pay special attention to vulnerable groups including older adults, individuals and families living with low incomes and newcomers to Canada, as well as those without access to primary health care services.
Reporting to the Clinical Manager, the Nurse Practitioner (NP) works as part of the inter-professional team with physicians, nurse practitioners, nurses, social worker, dietitians, and other staff in the delivery of high-quality Primary Health Care services to SVCHC clients. The NP provides primary health care services within the scope of the RN-EC including physical assessments, treatment, diagnosis and interpretation of test results, therapeutics, health promotion/education/counselling, history taking, screening, referral, and follow-up.The NP consults with physicians and/or other health care providers when a client’s condition requires care beyond the scope of practice (RN-EC).
