The primary focus is to implement and coordinate the Advance Care Planning initiative and to research, develop, and facilitate educational events including the development and dissemination of resource materials regarding Advance Care Planning. Education will be provided to members of the health care team across the continuum, contacts in surrounding health regions, and the general public. The incumbent is accountable to the Director of Clinical Standards & Professional Practice and works collaboratively with the Advance Care Planning Steering Committee. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"Human Resources Exemption: NoExperience Min 5 yrs exp w/end of life care & adult ed or community dev/health promotion Education Also accepted a degree in Health Administration or Education Undergrad degree in: social work, nursing, physical /cccupational therapy Competencies Communication, writing, presentation and interpersonal skills.   Demonstrates leadership skills Demonstrates word processing skills using Microsoft Office programs Knowledge and Abilities Competent critical thinking/judgement/autonomous decision making capabilities Demonstrates ability to apply research and theory to practice Demonstrated initiative, self-direction, organizational & leadership abilities  Knowledge in LEAN management system Develop/present educational training material to healthcare professionals Work collaboratively w/ a multidisciplinary team & other agencies in the commnty Coping skills to work w/persons dealing w/stress & other end of life issues Demonstrates strong commitment to "patient centered care" Demonstrates the ability to have a meaningful Goals of Care discussion Knowledge in population health promotion approaches and community development Other Information Please submit a cover letter and resume when applying for this position Preference to HSAS members as per the SAHO/HSAS Collective Agreement For more information on the city of Regina please visit https://www.regina.ca/ Registered/eligible for registration with the appropriate licensing body
