Education
Associates Degree of Nursing from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
Licensure
Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO. Must be licensed as a Registered Nurse. Certification by the American Nurses Credentialing Center (ANCC) as a Psychiatric and Mental Health nurse is preferred but not required.
Experience
A minimum of three years’ experience as a registered nurse in an ambulatory or inpatient setting with the ability to perform nursing assignments of moderate to severe difficulty and experience with acutely and chronically ill adults. A minimum of three years of experience with a variety of pharmacological agents used in patient treatment, including knowledge of the desired effects, side effects and complications of their use, as well as the accurate administration of the pharmacologic agent, particularly psychopharmacological agents. Must have at least 36 months of total nursing experience in a direct patient care clinical setting. Experience in mental health is preferred but not required.
Preferred Qualifications
- Experience in primary care clinics and/or care management.
- Experience in mental health.
- Experience working with patients with behavioral health symptoms.
- Experience working with active-duty military personnel, and/or experience working in military medical treatment facilities.
Summary
As a member of an integrated care team, the Registered Nurse - Integrated Primary Care Behavioral Health Care Facilitator (BHCF) facilitates continuity of care through a multidisciplinary approach in the treatment of depression, PTSD, anxiety disorders, and other conditions in adult beneficiaries, using a population-based care management model in support of primary care providers. Initiates routine calls / contacts to gather information to monitor health symptoms and severity using validated tools. Performs independent, comprehensive, telephonic, and in-person clinical triage of BH symptomology. Independently instructs and demonstrates to patients how to provide self‑care and access relevant services. Communicates with primary care providers and other providers regarding patients and documents events appropriately using electronic medical record.
