Mental Health Therapist (Pediatric Recovery Initiative )

Rexdale Community Health Centre is a non‑profit, multi‑service facility that offers primary health care, settlement, and community development activities for women, families, youth, adults, and seniors from diverse cultural backgrounds. The Centre is dedicated to delivering accessible and sustainable programs and services to diverse communities. It employs service delivery methods that incorporate advanced access principles, utilizing group visits and self‑management models. Currently, the Centre is seeking a Mental Health Therapist to collaborate with the mental health team in providing support and primary health care to Black children, youth, and mothers. The successful candidate will join the Centre’s Clinical team.

Job Summary

As part of Ontario’s pediatric Recovery Initiative, the Mental Health Therapist will provide culturally responsive, community‑based mental health care to Black children, youth, and families. This role supports a variety of innovative programs, including early psychosis intervention, mobile mental health clinics, and prenatal/postnatal care for Black mothers and children. This position will work collaboratively with interdisciplinary teams and community partners to improve access to mental health services and promote wellness in our community.

Responsibilities

- Conduct and deliver goal‑focused, trauma‑informed assessments and treatment in partnership with youth, families, and other supports, working toward improving functioning in the community and overall well‑being
- Provide one‑on‑one counselling, including psychosocial assessments, using frameworks such as the Social Determinants of Health framework (e.g. socio‑economic position, housing, educational, financial and legal needs)
- Deliver culturally safe, trauma‑informed, and anti‑oppressive therapeutic interventions, tailored to the client’s individualized needs
- Work closely with programs such as the Etobicoke Pre‑and Post‑natal Program.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Support early psychosis intervention and mobile mental health clinics in community settings.
- Maintain accurate clinical documentation by maintaining client records, assessment notes and correspondence within the safeguard of confidentiality
- Participate in quality improvement initiatives, program evaluation and reporting
- Engage with families and caregivers to support mental wellness and resilience.
- Participate in non‑police mental health crisis response initiatives for youth.
- Contribute to health promotion, education, and outreach activities in schools and community organizations
- Utilize a strengths‑based approach to empower youth and families in achieving their goals
- Provide case management and system navigation supports for youth, including working collaboratively with formal and informal support systems

Qualifications and Assets

- Master’s degree in social work or counselling/therapy discipline from a recognized university or equivalent with work experience
- Membership in good standing in the provincial regulating body of either the Ontario College of Social Workers and Social Service Workers or the College of Registered Psychotherapists of Ontario is required
- Minimum Three counseling/therapy experience, preferably in community based health setting with youth and their families
- Proven skills in psychosocial assessment, counseling/therapy, crisis intervention, case management and referrals
- Demonstrated skills and experience in working with youth of African descent
- Demonstrated commitment to and lived experience of anti‑Black racism and Afrocentric values and principles;
- Knowledge of community resources for African Canadian and Caribbean youth and their families in substance abuse treatment is essential.
- A deep fundamental and comprehensive knowledge of the unique challenges faced by African Canadian and Caribbean youth and their families across the social determinants of health.
- Ability to work with Black youth to develop and enhance their identity, culture and sense of belongingness to their community.
- A commitment to interdisciplinary team approach to coordination of health and social services
- Proficiency in current assessment, counseling/therapy and psychosocial counseling techniques for working with individuals, groups and communities
- Demonstrated skills and experience in the assessment and treatment of substance abuse and//or mental health treatment inclusive of individual and group counseling.
- Strong communication skills (verbal and written) combined with computer literacy are required. Ability to negotiate, communicate and maintain a supportive, trusting relationship with those being served.
- Experience in program development, implementation, and evaluation.
- Knowledge of community‑based health services
- Strong and demonstrated ability to form relationships with individuals while maintaining healthy boundaries
- Caseload management skills, including developing action plans for clients, monitoring client progress and maintaining records
- Working knowledge of the Ontario Mental Health Act
- Knowledge of Personal Health Information Privacy Act, Consent of Information / Records of Information and Medical Records Management
- Knowledge of the Rexdale community
- Bilingual in French and English considered a strong asset
- Must be able to provide a vulnerable sector security clearance

NOTE

- A criminal record check will be a required
- Rexdale Community Health Centre requires persons working with RCHC to be vaccinated

Salary Range

$65,327 to $78,704 annually (based on experience)

Contract End Date

March 31, 2025 with possibility of extension
