Description

As a Dietitian, you will provide dietetic and nutritional services to patients to optimise nutrition, promote overall health and well‑being, and develop individualized food plans to meet specific needs. Whether working in an office, community, or health‑care facility, you will play a lead role in providing connected, accessible, patient‑centered care.

What you will do

- Respond to referrals from Care Coordinators to administer nutritional and dietetic services to patients.

- Assess patients to identify nutritional needs and determine patient‑centered treatment goals.

- Provide treatment and nutritional care programs and develop resources to address patient dietary needs and achieve therapeutic goals.

- Oversee patient progress and adjust treatment programs as needed to ensure nutrition goals are met.

- Educate and counsel patients, families, caregivers, and SDMs regarding nutritional conditions, patient issues, and dietary strategies to improve function and nutrition.

- Manage service delivery in conjunction with the Care Coordinator and families, using all available forms of communication.

- Link patients with other in‑house services or outside organizations.

- Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected.

- Accurately document and chart patient assessment results, goals, treatment plans, and interventions.

What we require

- Registered Dietitian in good standing with the College of Dietitians of Ontario (CDO).

- Relevant experience providing nutrition and dietetic services in a community/health‑care environment.

- Comprehensive knowledge of dietetic services, practice methods, professional skills, nutrition principles and strategies.

- Strong interpersonal and collaboration skills to manage service delivery with Care Coordinators, patients and other health‑care professionals.

- Good understanding of the roles of other healthcare professionals and complementary therapies.

- Knowledge of funding agencies to assist patients in accessing needed support services and community resources.

- Practical knowledge of pertinent legislation.

- Comprehensive knowledge of the standards of practice and professional guidelines set forth by CDO.

- Ability to prioritise professional duties and manage caseload efficiently.

- Ability to accurately complete required documentation, reports, forms and recommendations.

- Effective listening, observation and facilitation skills.

- Clear communication skills across meetings, reports, letters and presentations.

- Working knowledge of computer software (email, internet) and Microsoft Office (Word, Excel).

- Valid driver’s licence and access to a vehicle.

- Advanced oral and written proficiency in English and French is essential.

- Bilingual candidates with advanced oral and written proficiency in English and French will be given first consideration; less qualified candidates who do not meet the language requirement may still be considered.

- Mandatory COVID‑19 vaccination policy – proof required prior to start date.

Preferred qualifications (“edge”)

- Experience working with diverse patient groups (multicultural, homeless, palliative, acquired brain injury, mental health, geriatrics, pediatrics).

- Experience with wound care, post‑operative GI surgeries, motivational interviewing, diabetes, oncology, gut health, food insecurity and chronic disease management.

- Case‑management experience or recent related community experience.

- Ability to speak French or another second language.
