Enhance community health as a Social Worker with Vancouver Coastal Health at Pemberton Health Centre, BC. Focus on client-centered care and interdisciplinary collaboration in this pivotal role.
This Social Worker position encompasses holistic mental health and substance use perspectives. You will work with a diverse client caseload, conducting assessments and implementing care plans to address social determinants of health. You'll utilize your knowledge of trauma-informed practices to facilitate recovery and support.
Key Responsibilities
• Provide psycho-social assessments and care planning
• Implement direct care in collaboration with healthcare teams
• Address complex client needs including mental health challenges
• Maintain holistic practices with cultural sensitivity
• Document client interactions and care plans effectively
Requirements:
• Master’s Degree in Social Work with 2 years related experience
• Active registration with the BC College of Social Workers
• Possess a valid BC Driver's License
• Expertise in mental health care and addiction services
