Invicta Health in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT) are seeking to recruit Community Mental Health Practitioners to be part of Mental Health Together.
What is Mental Health Together?
In Kent and Medway, the community mental health transformation model has created a new service called Mental Health Together.
It is a multi-agency offer, which means bringing all the services and agencies that support people with varying mental health needs much closer together. This ensures that people get the right care, without having to navigate service boundaries and without repeatedly telling their story.
Mental Health Together will add new roles, as part of a new joined-up model of care which bridges the gap that currently exists between primary (i.e. GPs) and community mental health (i.e. KMPT). It will treat people with complex mental health needs by:
- Providing someone with support in a person-centred, holistic way
- Removing barriers to access, and thresholds across services working together
- Involving service users and carers as equal partners in their care
- Building in involvement and co-production at all levels of service design delivery
- Delivering needs-led and personalised trauma-informed care.
Main duties of the job
The Registered Mental Health Nurse / Occupational Therapist will be expected to deliver stepped care by delivering a range ofevidence-based interventions which will promote recovery, social inclusion andnormalization for service users who suffer from mental health problems.
They will work in partnership with the MDT, CRHT, Acute Inpatients Services,other community teams, Primary Care and the Voluntary Sector to provide supportand treatment to service users and facilitate the safe management of theirneeds within a range of community settings.
Theprinciple purpose is to promote mental and physical health and prevent illhealth by treating mental health problems effectively through evidence basedinterventions that build on strengths and supports choice and is underpinned bya single care plan within a model of care based on inclusivity, particularlyfor those people with co-existing needs, who experience marginalisation
They will be a Registered Mental Health Nurse or Occupational Therapistresponsible for patients allocated to them and will be involved in assessment,management planning and delivering care.
The post will support their peers and team leaders and be an effective teammember whilst working on their own initiative.They will supervise Band 5 Nurses/OTs and pre-registered students andhold a mentoring qualification. They will have a commitment to collaborativeworking and actively supporting and liaising with other health and socialprofessionals.

- Be proactive, innovative,and creative in delivering care, enabling the facilitation of service usersrecovery.
- Engage service users and carers in relapse prevention and riskmanagement /contingency plans, providing interventions that maximize theservice users ability to resolve relapse crisis and prevents admission tohospital.
- Liaise with inpatient units to support discharge through appropriatepathways.
- Facilitate early discharge from both inpatients and the CRHT Team byproviding regular, formal reviews.
- Deliver aflexible and responsive service through practice that is dictated by serviceusers/carers needs.
- Workcollaboratively and effectively with members of the multi-disciplinary team andin partnership with other agencies.
- Deliver high quality care and provide solutions and signposting toservice users with severe and enduring mental health problems to supportcorrect pathway access.
- Provide carewithin the Strength Based Model of practice maintaining service users withintheir own environment in the community.
- Act as aneducator for service users and carers.Advise on issues of mental health to generate a positive approach tohealth and strengthen the service users capacity to manage illness, symptoms, and relapse.
- Respond swiftly to urgent referrals providing interventions to preventadmission or re-admission.
- Work closely withcarers.
- Recognise racial,cultural, sexual preference and address health inequalities.
- Ensure thatrecord keeping/documentation reflects needs.
- Maximize the useof resources within the team and available in the community.
- To be responsiblefor assessing, planning, implementing, and evaluating care, usingevidence-based tools and models of care.
- Work within theMDT framework according to code of practice, for delivery of optimum care.
- Ensure that careplans are reviewed and updated to take into account of the most recentinformation regarding service users needs.It should reflect the views of the service users and carers.

- Ensureactivity/contact is entered promptly on RiO, conforming to the organisationsdata quality requirements, in line with contract specification Trust and SocialCare data quality requirement.

Clinical Skills
- Have a workingknowledge of current appropriate legislations.

Training

Person Specification
Experience
- Minimum of 2 years post registration experience.
- Previous experience of working in a community team.
Qualifications
- Registered Mental Health Nurse, or registered Occupational Therapist (holding diploma or degree qualification).
- MELPS (Mentoring and Enabling in a clinical setting).
