**Older Adult Support Worker & Advocate**

Camrose & District FCSS is seeking a dynamic individual to provide case management for older adults experiencing elder abuse in the community of Camrose, County of Camrose and the city of Wetaskiwin. Case management includes risk and safety planning, screening, intake and assessment, service planning, case conferencing and consent & disclosure process. The older adult support worker and advocate must use best practices, follow ethical guidelines and respect the individual's uniqueness and right to be protected. The older adult support worker and advocate works closely with a multi-disciplinary team to provide exemplary case management. This is a full-time, permanent position, working 35 hours/week, Monday to Friday - with the understanding that flexibility is required.

**RESPONSIBILITIES**

The Older Adult Support Worker and Advocate is responsible, under the direct supervision of the Program Director, for the direct service delivery including:
**Program Planning, Monitoring and Evaluation**
- Works with the Program Director to plan/design, implement, monitor, and evaluate the program for older adults
- Screening inquiries as to needed for intervention; contact police/RCMP in emergent circumstances or potential criminal matters.
- Refer non-elder abuse inquiries to appropriate community resources.
- Conduct intake and assessment interviews with potential clients
- Create and implement individualized service plans for clients requiring on-going support.
- Advocate on behalf of clients accessing services and supports.
- Coordination of services with community supports
- Compile reports for Alberta Elder Abuse Awareness Council as well as Camrose & District FCSS
- Complete various surveys and evaluations with clients, service providers and key stakeholders.

**Operational and Financial Management**
- Assists the Program Director with financial planning and administration of the program.
- Assist Program Director to prepare various proposals and/or fund-raising requests, for elder abuse programs, as necessary.

**Client and Public Relations**
- Promotes and practices excellent public relations with clients, program participants, their families, other Camrose & District FCSS clients and personnel, and the community in general.
- Provides education and training for volunteers regarding elder abuse service delivery practices and especially stressing respect for the needs of clients, program participants and their families.
- Providing public education -deliver presentations to interested groups; provide the public with updates on elder abuse

**Community Partnership and Liaisons**
- Cooperates with other community organizations serving older adults.
- Develops and maintains effective communication links with related organizations.
- Collaborate with the coordinated community response team to provide education, awareness and support.
- Participation in monthly community of practice calls and educational opportunities offered by the council to case managers and/or the grant recipient
- Facilitating communication between supports to optimize efficacy
- Be an ambassador within the community of seniors services, liaison with agencies as needed about client and potential needs and work collaboratively with support agencies or organizations.
- Supporting capacity - support volunteer education and ongoing development; share resources.

**KNOWLEDGE, EDUCATION AND EXPERIENCE**
- Degree or diploma in a human service field, or equivalent education and experience
- Minimum two (2) years experience working in human services field
- Ability to provide compassionate, client-centered, trauma-informed services
- Excellent interpersonal, inter-professional team skills, and oral and written communication skills
- Superior problem-solving skills, organizational and planning skills
- Comfortable working independently; available some weekends and/or evenings
- Vulnerable Sector Police Information Check required;
- Valid Alberta Driver’s License, reliable vehicle with appr
