**Dietitian**

**(Permanent Full-Time)**

Reporting to the Manager of Allied Health, the Dietitian is responsible for providing quality nutrition counseling to admitted inpatients, community-referred outpatients, including but not limited to the Diabetes Education Program. Working in collaboration with an interprofessional team, the Dietitian will share expertise and knowledge of nutrition care with members of the health care team.

**DUTIES AND RESPONSIBILITES**
- Assists in the investigation and follow up of changes to the patient’s nutritional status
- Provides education to patients and families regarding nutrition and related topics.
- Maintains pleasant rapport with patients at all times
- Acts as a resource to members of the interprofessional team in the provision of nutrition and diet information.
- Participates in the interprofessional team, as assigned,
- Participates in patient care rounds and in the respective team.
- Maintains records as required by the Hospital and the College of Dietitians of Ontario
- Counsel’s patients as required and utilizes clinical information obtained from the patient/family members/healthcare team and patient chart.
- Develops and modifies nutrition/diet plans as required according to patient needs.
- Provides advice on menu and food preparation.
- Performs TOR-BSST screening to identify dysphagia risk.
- Works in collaboration with the Diabetes Education Program nurse to support patient education and care.
- Assists with the development of policies and procedures related to clinical nutrition and the Diabetes Education Program e.g. EN, dysphagia, therapeutic diets.
- Assists in the development and maintenance of the formulary for nutritional supplements.
- Attends and contributes to medical rounds, conferences, and lectures.
- Attends and participates in departmental meetings.
- Participates on committees as required and promotes professional / personal development.
- Participates in department goal setting, evaluation, and planning.
- Participates in self-directed learning to ensure that nutrition practice remains relevant (attending professional conferences, telemedicine, journal reviews, hospital rounds).
- Meets College of Dietitians of Ontario continuing education requirements.
- Appropriate time management/prioritization of patients
- Reporting of clinical statistics as required by the Hospital.
- Provides in-services to staff regarding dietary/nutrition services.
- Provides education to interprofessional team members.
- Maintains an appropriate level of in-service and information as required by the hospital.
- Contributes to department by performing related tasks, as required.
- Models CMH philosophy and values.
- Maintains knowledge of CMH policies and procedures.

**EDUCATION, EXPERIENCE, SKILLS & ABILITIES**:

- Registered Dietitian with the College of Dietitians of Ontario, in good standing, and membership with Dietitians of Canada.
- Two years’ experience in a health care setting preferred.
- Certification as a Diabetes Educator preferred, or willing to work toward.
- Demonstrated ability to provide excellent customer service when dealing with patients, volunteers and other members of the community and public
- Demonstrated pattern of consistent attendance and capability of maintaining that same standard.
