**The Role**:
Are you a patient focused Lab Technician with medical administration experience and excellent written and telephone communication skills?

KMH Cardiology Centres Inc. is growing and we are recruiting for a Patient Care Coordinator (PCC) for our busy specialist physician practice in **Markham.**

The PCC is responsible for providing empathetic and efficient direct or indirect care for patients attending KMH labs:
Under physician direction the PCC Lab Techs:

- Complete patient intakes
- Serve as a liaison between members of the multi-disciplinary team to ensure:

- High standards of quality and optimal management of patient care outcomes
- Ongoing continuous improvement practices to support clinic processes

**The Accountabilities**:

- Patient Intake
- Coordination of daily clinic operations
- Management of Physician orders
- Tasks Management
- Respond to patient enquiries regarding clinical care within 24 business hours
- Manage missed appointments and demonstrate ability to empathize with the complexity and difficulty of each patient when following up on missed appointments
- Inform and educate patients about tests and procedures as requested by physician
- Process prescription renewal requests as directed by physician in collaboration with administrative staff within 24 business hours
- Foster a collegial, respectful clinic environment

**Minimum qualifications**:

- Lab Technician (certified)
- Medical administration experience

**Required skills and experience**:

- Proven ability to work proficiently in a busy fast paced specialist clinic environment
- Self-directed, detail oriented with an ability to multitask, establish priorities and achieve objectives with mínimal supervision
- CPR
- Strong knowledge of medical terminology
- Excellent verbal and written communication skills
- Superior telephone etiquette
- Strong conflict resolution skills
- Proficiency in technology including MS Office and EMR systems experience
- Excellent time management and organizational skills
- Possesses effective analytical and problem solving skills
- The ability to provide coverage at both locations
- Able to deal with sensitive information with a high degree of diplomacy
- Team orientation

The Patient Care Coordinator team takes pride in our patient focused environment and works very closely with other service and clinical teams at KMH.

This is a full-time role scheduled for 5 days and 37.5 hours per week (between _Monday-Saturday_) during the hours of 8.00 a.m.6.00 p.m.

The role is based at our Markham lab. This position reports to the Team Lead: Patient Care Coordinators.
