**JOB SUMMARY**

This position is responsible for coordinating and delivering support services to residents by assisting them to achieve their goals and reach their personal potential.

**KEY DUTIES & RESPONSIBILITIES**

Provide Case Management services to residents, helping residents to maximize their ability for self-determination by enabling them and empowering them in decision-making, and assisting with goal setting, planning, and follow through of identified goals;
- Assess residents’ relevant skill strengths and development needs;
- Connect residents with appropriate community services by assessing their need and acting as a liaison between residents and agencies;
- Provide counselling as needed to residents;
- Act as an advocate for the needs of the residents of The Madison;
- Maintain client records in a timely manner;
- Monitor, evaluate and communicate progress of residents with the residents themselves, team members and the supervisors on a regular basis.
- Provide tenant and eviction prevention services;
- Assist residents in determining their strengths and interests and pursuing vocational related activities;
**EDUCATION & EXPERIENCE**
- Post-secondary degree/diploma in Social Sciences a combination of education and experience may be considered;
- Experience in case management in the social services field with an eminence on assessment, service planning, evaluation, follow up, review and closure;
- Experience dealing with individuals living with mental health issues and addiction;
- Must have a valid class 5F driver’s license and access to a vehicle in good condition and be willing to drive residents to appointments;
- Experience in Non-Profit Sector is an asset;
**KNOWLEDGE/SKILLS/BEHAVIORS**
- Demonstrated knowledge in community resources available for homelessness, health, mental health, and substance use;
- Knowledge of Housing First and Recovery-Based housing models;
- Case Management skills;
- Good understanding of colonization and its impact on Indigenous peoples;
- Comprehensive knowledge of client centered approach;
- Strong written and communication skills;
- Proficiency in Microsoft Office Suite and Database platforms;
**RELATIONSHIP**
- Interaction with residents, volunteers, external agency partners, and staff;
**WORKING CONDITION**
- Hours of work will be Monday-Friday, 08:00am-04:30pm with occasional weekend and evening hours based on participant need
- Hazardous conditions could potentially include unpredictable or risky behaviours from community members;
- Must have a valid Class 5F driver’s license, a clear driver abstract, a vehicle in good condition, and be willing to drive participants to appointments;
- May be required to lift up to 50 lbs. occasionally and assist participants with moving related to housing;
