**KEY FUNCTION**:
Provide spiritual and religious care, and assess spiritual and emotional needs of patients, families, staff, and volunteers. Ensure practices are consistent with the mission, vision, and values of HSN.
**REPORTING**:
Under the general direction of the Clinical Bioethicist/Manager Spiritual and Religious Care.
**DUTIES**:
- Provide spiritual care to patients, families, staff, and volunteers, while providing Chaplain-initiated services on all units through empathetic listening.
- Conduct spiritual assessments and document all visits in patient charts according to professional College guidelines.
- Practice evidence-based care, including ongoing evaluation of new practices.
- Organize, design, and lead religious ceremonies of worship and rituals.
- Identify and arrange for patient’s spiritual practice to be met within a multi-faith, no faith, or multi-cultural context.
- Organize and design spiritual and emotional wellness groups on in-patient units.
- Maintain confidentiality in accordance with the Personal Health Information Protection Act (PHIPA), Canadian Association for Spiritual Care (CASC) and CRPO.
- Identify and clarify ethical issues, and provide support and guidance in the ethical decision-making process to ensure adherence to HSN’s ethical decision-making framework.
- Act as an advocate for spiritual and religious care needs of patients, families, staff, and volunteers.
- Educate staff on the role of Spiritual and Religious Care and other in-services as requested.
- Act as a resource to staff regarding diversity sensitivity.
- Seek and create opportunities to enhance the quality of professional care practice.
- Determine and align improvement projects with HSN’s Strategic Plan, monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
**QUALIFICATIONS**
**EDUCATION AND TRAINING**:
- Minimum Master’s Degree in Religious Studies, Theology, or in a related field.
- Minimum of one (1) Clinical Pastoral Education (CPE) unit or equivalent.
- Current Certificate of Registration with the College of Registered Psychotherapists of Ontario (CRPO); or the College of Nurses of Ontario (CNO); or the College of Occupational Therapists of Ontario (COTO); or the College of Psychologists of Ontario (CPO); or the College of Physicians and Surgeons of Ontario (CPSO); or the Ontario College of Social Workers and Social Service Workers (OCWSWSSW).
- Membership with Canadian Association for Spiritual Care required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
**EXPERIENCE**:
- Minimum of two (2) years’ experience working as a Spiritual Care Professional within a health care environment.

- Demonstrated knowledge of current practices and theories through integration into the provision of Spiritual and Religious Care.
- Demonstrated ability to work collaboratively as a member of the spiritual and religious care team.
- Demonstrated ability to facilitate groups,workshops, educational programs, presentations, and in-services.
- Demonstrated ability to communicate effectively with patients/families and other Health Care Professionals.
- Demonstrated ability to understand constructive input and adapt practice to reflect new learnings.
- Demonstrated ability to function in a multi-faith environment with equity, justice, and sensitivity.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated strong organizational skills with the ability to prioritize workload.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
- Demonstrated knowledge of medical terminology.
- Demonstrated knowledge to mentor students and support personnel.
- Demonstrated knowledge of current trends
