**Day Support Worker**
**Position Type**:Part-Time - 30 hours per week
**Location**:Burnaby
**Compensation**: $25.95 per hour - as per Community Living Collective Agreeme
**Requirements**:Must have a drivers license and vehicle with Business insurance, please indicate this on your resume
To facilitate meaningful and enriching daytime activities for adults with developmental disabilities, focusing primarily on building social connections and belonging in the community through partnerships and volunteer work, and by which persons served can be contributing members of the community. To foster community relationships by building on abilities of persons served.
**SCHEDULE: 30 hours per week**
Sunday: Day OFF
Monday: 9am-3pm
Tuesday: 9am-3pm
Wednesday: 9am-3pm
Thursday: 9am-3pm
Friday: 9am-3pm
Saturday: Day OFF
**YOUR RESPONSIBILITIES**:
- Participates in the assessment, goal setting, and service planning of persons served. Documents, implements, and provides input into the evaluation of persons served’s programsParticipates in the development, implementation, and on-going documentation of persons served Plan of Care (e.g. Health Care Plan, Behavioural Support Plans, Individuals Support Plans, etc.)
- Identifies a persons served’s interests and abilities including cultural and religious factors.
- Identifies and builds social connections and belonging based on a person’s interests and abilities.
- Facilitates relationships and connections wherein persons served expand their personal support networks and find new and meaningful relationships.
- Facilitates growth and independence of persons served so that it reflects natural levels of social interdependency. Established November 14, 2003 Revised May 23, 2013 Page 2 of 2
- Works towards achieving Community Engagement goals.
- Recognizes, analyses and deals with urgent or emergency situations and ensures that the health and safety of persons served is closely monitored and risks mitigated. Reports concerns to the SSW/Team Leader in a timely manner.
- Maintains financial records of persons served and program according to established procedures.
- Is knowledgeable of and encourages each person served to develop and expand his/her methods of communication.
- Participates in all aspects of the Health and Safety Program including safety and emergency procedures, in order to provide a safe work environment. Reports all concerns to the SSW/ Team Leader.
- Performs other duties as required.
**YOUR QUALIFICATIONS**:
- A diploma in human services or a related discipline or an acceptable combination of education and experience
- Minimum one year experience in working with people with developmental disabilities and/or physical/medical/behavioural support needs
- A strong commitment to the philosophy of people with disabilities living and working in the community
- Experience and skills related to the Program’s mandate as required
- Demonstrated ability to work within a team setting, with an emphasis on leadership, self-initiative, maturity and tact
- Good written and verbal communication skills
- Good organizational skills
- Demonstrated teaching and coaching ability
- Knowledge of the local community’s resources and services
**YOU MUST HAVE**:
- Drivers License and Clear Driver's Abstract
- Criminal Record Check for Children and Vulnerable Adults clearance
- Current CPR and First Aid Certification
- Current negative TB Test
- **Your own vehicle with $3 million liability business insurance**
