**Working Schedule**
Typical working schedule is Monday-Sunday (Assignment based on program/organizational need). Includes daytime and/or evening hours but not overnights; typically includes weekdays and/or weekends and may include statutory holidays with advance notice)

**Remuneration**
- $33.22 hourly rate
- Healthcare Spending Account, which can commence 1-year post-probationary period

- Provide formal mentorship to staff members throughout the entire employee lifecycle, which includes direct and constructive feedback, encouragement, coaching and knowledge sharing on a daily basis
- Execute coaching objectives as per Managers’ direction
- Provide critical information and feedback to Manager on a bi-weekly basis on individual performance trends/themes that arise that could serve as an avenue for coaching, development and refinement
- Alert Manager on any issues that could have a direct impact on quality care and provide solutions on how to respond and mitigate the existing challenges
- Maintain a higher acuity clinical profile as a sign of your elevated skill set and expertise
- Co-facilitate clinical team meetings or be the designate facilitator in the Managers’ absence
- Contribute to policy development and/or revisions when required
- Provide consultation to leadership team when required on key initiatives in the organization on a bi-annual basis
- Lend a clinical lens and perspective to internal and external committees’ and stakeholders as required
- Contribute to the development of the program content and schedule
- Provide insight and recommendations on how processes involved in programs and services and community outpatient department could be improved
- Oversee the execution of synopsis and communicate relevant information to Manager and live-in treatment team
- Manage the medication management processes upon confirmation of service recipient entry
- Co-design treatment plan, transitional plan that is aligned with client assessment, presenting problem and desired outcome
- Provide/facilitate group and individualized clinical programming/sessions
- Facilitate mediation and conflict resolution when required
- Develop action plans, implementation and follow-up with the client on the routine basis to support the client treatment plan
- Provide psychoeducation that meets the client where they are at and offer adaptive strategies/coping skills focusing on mindfulness, distress tolerance, emotional regulation and interpersonal effectiveness
- Liase with primary care, community treatment organizations and resources to provide necessary support to the client
- Advocate and promote client-centred care in all affairs
- Utilize client routine outcome monitoring data in the development of individualized sessions
- Attend staff meetings and actively participate throughout
- Analyze assessment prior to client entry and adequately prepare the delivery of high quality and individualized counselling services
- Conduct high-quality, evidence-based interventions to promote health and well-being and are relevant to Addiction and Mental Health population
- Execute timely and accurate documentation that adheres to the organizations’ policies and procedures and regulatory college/body; maintain routine progress records
- Participate in the research, development and implementation phases of program material and delivery
- Provide colleagues with relevant information to assist in the delivery of services in a congregate setting
- Participate in clinical and organizational meetings for the purpose of delivering high-quality addiction treatment and contributing to the advancement and refinement of processes, programming and operations
- Participate in organizational initiatives that support primary purpose, quality implementation and strategic activities
- Participate in ongoing learning to advance skills, knowledge and competency
- Provide feedback during the process of accreditation and contribute to policy analysis and development when required
- Correspond and consult with treatment team and contribute to effective internal communications that promote effective treatment services
- Connect and secure client-driven referral sources to aid in the individuals’ transitional plan upon termination of services
- Actively build, sustain and promote Hope Place Centres programs and services
- Ensure Manager is informed and updated on critical issues, incidents and/or complaints in a timely manner
- Perform on-call responsibilities on a rotation to provide consultation, urgent support and/or secure coverage for program delivery in order to preserve and maintain
