**Responsibilities**:
The Family Support Liaison leads the development and execution of activities for the family support program within Legacy of Life/Critical Care Organ Donation. S/he is a resource for professional colleagues in matters pertaining to the care of donor familier and ensures appropriate and timely family follow-up through the delivery of a 12-month donor family aftercare program.

This role provides advice to internal and external stakeholders in disseminating standards and guidelines for evolving improvement approaches with donor families. This position is responsible for the development of a post-donation family support plan including family exchanges with anonymous and/or identified donors as well as meetings between donor families and recipients.

The Family Support Liaison leads proactive initiatives to improve the experience of donation for family members, including forging effective partnerships with all health care team members who interact with the family during the donation process, and solicits feedback from donor families to evaluate their experience with the donation process. The Family Support Liaison ensures consistent and effective implementation of best practices in approaching potential donor families.

By working with health care partners and stakeholders, province-wide and nationally, this role will ensure that the option of organ donation, as an integral part of standardized quality end-of-life care, is offered to the family in a compassionate, timely, and informative manner.

This role will also work closely with stakeholders and Nova Scotia Health Authority (NSHA) communications team in the development of a communication plan to increase public support of organ donation and to provide ongoing support for organ donor families.

**Qualifications**:

- A Bachelor’s Degree in Social Work from an accredited and approved School of Social Work/University Program with current registration with the Nova Scotia College of Social Work or equivalent (e.g. Master’s of Divinity from a recognized institution accredited by the Association of Theological Schools)
- Experience in providing development of education, program implementation and evaluation required
- Demonstrated 3 years’ experience in working with patients and their families going through the grieving process and effective bereavement support strategies an asset
- Demonstrated professional experience working with marginalized or socially disadvantaged communities within the healthcare setting an asset
- Passionate about achieving excellence in the provision of family support services, before, during and after deceased donation
- Experience with the grieving process, effective bereavement support strategies and the unique needs of donor family members
- Ability to provide care that is sensitive to the patient and family's personal and cultural values, beliefs and practices, their developmental stage and, readiness to address matters associated with a life-limiting illness, death and bereavement
- Demonstrated awareness of and sensitivity to varying religious, cultural and ethnic influences on end of life care and decision-making
- Demonstrates sensitive, compassionate family care; able to project these attitudes in an influential way to their peers
- Skilled in collaborative conflict and/or concern resolution
- Exceptional organizational and public relations skills
- Exceptional interpersonal skills, including oral and written communication
- Proven critical thinking, organizational and communications skills, including writing and editing
- Experience in adult education principles
- Excellent interpersonal, presentation and group management skills
- Demonstrated high degree of independent decision-making, self-direction, motivation, change management knowledge, time management and team building/leadership skills
- Demonstrates experience providing family care and support in crisis
