Radius Community Health & Healing provides interdisciplinary, team based, integrated, comprehensive, and person-centered primary health care for vulnerable Edmontonians with complex needs. Through primary care and health promotion, Radius Community Health & Healing improves the quality of life and well-being of populations and individuals who face multiple barriers to accessing health services. Radius achieves this by providing the highest standard of evidence-informed and client focused inter-professional care. We collaborate with our clients to address the broad determinants of health.

The Recovery Coach aims to assist individuals in accessing Radius programs through their recovery process by offering client-focused, culturally appropriate services. This includes an individualized assessment, treatment plans, case management, crisis intervention, education, and referrals. Their role is to guide and motivate clients to access mental health, physical health, substance use, life skill, and other supportive services to enhance autonomy and overall quality of life.
- Key Responsibilities_
- Meet clients where they are at in their recovery process and help create individualized recovery plans. As well as helping clients to overcome barriers that may be preventing their recovery.
- Help clients to build motivation to accomplish goals; also monitor and evaluate client progress and achievement of goals and adapt plan as needed.
- Develop working relationships with a network of community stabilization service providers in the fields of housing, health, mental health, and substance abuse.
- Work collaboratively with other health and service providers and provide referrals and warm hand offs as appropriate.
- Provide follow-up services and care as needed.

Qualifications_**
- Registration with Alberta College of Social Workers (ACSW) is preferred.
- Current Basic Cardiac Life Support - Healthcare Provider (BCLS-HCP)
- Standard First Aid training
- Mandatory recent vulnerable sector check and police information check.
- Mandatory experience with marginalized populations (e.g. homeless serving sector, social service agencies)
- Certificate from a Recovery Coach program or willingness to work towards certification is an asset.
- Non-Violent Crisis Intervention training is an asset.
- Mental Health First Aid and Applied Suicide Intervention Training are considered an asset.
- A valid class 5 driver's license and be able to reliably commute between work locations.

- Lived experience considered an asset.
- Excellent relationship building skills for use with clients, colleagues, and community agencies.
- Able to balance numerous demands and conflicting priorities, strong problem-solving skills.
- Ability to work co-operatively within a multidisciplinary team.
- Ability to demonstrate patience, understanding and empathy towards those with addictions and behavioral disorders.
- Strong computer proficiency, with experience with Office 365 suite (e.g. Outlook, MS Office, Excel)
- Prior experience with electronic medical record (EMR) system an asset (e.g. MedAccess)
- Excellent written and verbal communication skills.
- Have a strong understanding of trauma-informed care, and must be able to operate within a harm reduction model of care.
